The first appointment with our office will be a time to address any concerns that you may have. We’ll help you with forms and do our best to answer your questions. Once we’ve accomplished that, you can relax and have a cup of Starbucks Coffee from the office beverage station while you take in the calming ocean scenery that is broadcast across our widescreen television in the reception area.

We encourage all patients to bring along insurance cards, a list of current medications, and any recent x-rays or referrals from another office. Please also download, print, review, and fill out the following patient forms:
